CPA 2001 Proceedings

Does anyone have a spare copy of CPA 2001 proceedings?  I’m after a specific paper: Towards a Viable Alternative to OO -- Extending the occam/CSP Programming Model by Tom Locke, and it’s not available through WoTUG.  Either a copy of the proceedings, paper, or how to contact the author would be helpful.
